The Levy Advice form contains information Coal LSL requires to accurately maintain records of an eligible employee’s entitlement.
All employers of eligible employees must complete a monthly Levy Advice form, due on the 28th day of the following month.
We make periodic improvements to the form to ensure it contains all the information we require and remains in the format needed in order for our systems to read and process the file.
This is why employers are required to download the form each month from our website or Levy Loader (if you are enrolled) rather than using copies of previous forms submitted.
